I just tried this with Acrobat pro version 2023.001.20174, which is an old version of acrobat. I wasn't getting the alt-text bug with any export method.
I updated to 2023.003.20201. I can now replicate your alt-text bug.
It occurs only when you use the "Save as Adobe PDF" option in word, which runs your document through the Adobe PDF Maker program.
If you just "Save as" your word document as a PDF, the alt-text bug does not occur.
